Your Specific Responsibilities:

Prepares regular preparation of programmatic data reports for Program Operations Administrator for monthly submission.

Conduct regular quality assurances activities including electronic client chart reviews, electronic program management reviews and monitoring client management and program development.

Coordinates and conducts timely personal interviews and re-interviews of individuals who have acquired HIV and provides Health Literacy support so that they are better able to equip themselves with the necessary tools to identify and understand what barriers to care they may be facing maneuvering through the HIV continuum of care.

Coordinates with local drug treatment facilities, mental health organization and other high-volume venues that provide service to high-risk clients, to provide timely screening for HIV and ensure appropriate referral treatment and follow up on care.

Assists with the planning of community presentations and producing of media advertising for HIV Education and Prevention, Early Interventions Services, Linkage to Care services to the community and for community health staff; planning and directing of a comprehensive HIV/AIDS and STD health education program; facilitating and participating in HIV/STD health education and prevention activities.

Participates in local prevention campaigns as assigned.

Keeps stock of age/gender/population specific HIV/STD education and prevention materials for FDOH, Brevard’s Community Health office, Clinics, and community organizations.

Required Knowledge, Skills, and Abilities:

Knowledge of and experience with computers and computer software: MSOffice

Knowledge of and experience with methods of data collection, analysis, and evaluation.

Experience in adherence counseling.

Experience working independently.

Experience with client confidentiality.

Ability to work with clients in the field.

Qualifications:

Minimum

Direct healthcare client management experience.

Experience with electronic client record management.

Experience with electronic report management: data compilation, measurement, and evaluation

Working knowledge of MSOffice/Excel

Current Florida Driver’s License

Ability to communicate with diverse populations: MSMs; LGBTQ+s; IDUs/ other substance abusers, homeless/unhoused, and sex workers.

Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an

emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters,

or performing other emergency duties including, but not limited to, responses to or threats involving any disaster

or threat of disaster, man-made or natural.

Preferred

This position requires client management and adherence counseling across multiple healthcare/sociologic disciplines.

This position requires client management out in the field with homeless/unhoused, and non-adherent individuals.

This position requires knowledge of event organization.

Incumbent must be able to demonstrate the proper use of condoms and discuss safer sex practices with diverse populations

Incumbent may need to demonstrate process of safe cleaning of syringes for HIV prevention among IDU population.
