The Options Counselor will educate older adults, persons with disabilities & individuals who support them to understand the type & range of community supports available. Duties & Responsibilities: The OC counseling includes meeting with the consumer to discuss & provide decision support about the consumer?s medical, physical, emotional, housing & financial resources. Also, what public, private & informal supports are available to meet their needs. Provide counseling on the different options available for long term care, supports & services. Counseling can be provided by phone, email or in person in a variety of settings including consumer?s home, in the office, a hospital or skilled nursing facility. Under the Waiver program, the OC will assist older adults & persons with disabilities to transition out of long-term care facilities by assisting & completing all documentation in the transitioning & coordination of services, housing & resources available. The OC will assist with the Mass Health application process, provide Medicare Plan Finder support & complete the Reginal SHINE Training, re-certifications & ongoing SHINE trainings. Assist the consumer in developing a plan that is consumer directed & includes services, program, financial resources, & other supports selected by the consumer. Provide timely & accurate response & assistance with referrals. Assist in marketing, community presentations & conduct outreach in a variety of settings & locations on a regular basis. Attend meetings as required. Other duties as assigned. Skills & Abilities: Ability to collaborate, take initiative, provide/ receive feedback. Ability to communicate & work with the elderly & community resources serving the elderly & disabled population. Ability to work independently & professionally. Excellent computer skills required. Valid MA driver\'s license & car required Education, Licensure & Experience: A Bachelor\'s degree in social work, human services, nursing, psychology, sociology or related field from an accredited college or university. Physical Requirements: Prolonged periods sitting at a desk & working on a computer. Must be able to lift up to 15 pounds. Travel made daily throughout our service area. Occasional travel to the Islands & off-cape.
