Amergis LPN/LVN Long Term Acute Care (LTAC) in Indianapolis, Indiana

Amergis Healthcare Staffing is seeking a Long Term Acute Care LPN/LVN. The Long-Term Acute Care LPN/LVN provides the delivery of direct andindirect patient care to medically complex patients under the clinical supervision of the RegisteredNurse.

Essential Duties and Responsibilities:

  • Provides delivery of direct and indirect patient care

  • Responsible for providing care to medically complex patients

  • Documents all activities related to the patient’s care according to thepatient’s plan of care

  • Maintains confidentiality at all times with patients, medical staff andco-workers

  • Provides a safe, comfortable and therapeutic patient environment

  • Experience in acute rehabilitation for vents, wound care, cardiac,stable post trauma patients

  • Adheres to and supports the policies/procedures/goals/objectives of thehospital in an attempt to provide quality patient care Revises plan of care according toevaluation, changes in medical plan of care and nursing interventions

  • Performs interventions according to identified priorities, plan ofcare, and the hospital policies and patient care outcome standard

  • Identifies and prioritizes actions based upon patient care requirements

  • Supports professional practice by observing the following: Usesknowledge and practices in accordance with the Nurse Practice Act; Joint Commission, state andlocal laws; and Patient’s Bill of Rights hospital policies.

  • Performs other duties as assigned/necessary

Minimum Requirements:

  • Current nursing license for the state in which the nurse practices in

  • One (1) year nursing experience in a long term acute care facilitypreferred

  • Ability to effectively elicit/provide information to and fromappropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strongcommunication skills; proficiency in the English language may be required

  • Computer proficiency required

  • Must be at least 18 years of age
